Community Living Support

Supports and services are tailored to individual needs and goals. These are identified in the individualized plan. This plan is developed through Person-Centered Planning. It involves the individual, family, and other key people. This can range from 24-hour support to periodic or monthly visits.

Supportive Independent Living

This program provides various personalized supports. These supports are for adults who:

• live on their own or with limited assistance
• live in the community outside their family home
• need help and guidance with daily living activities such as:
  • grocery/personal shopping
  • meal preparation
  • money management
  • household tasks

Skill development training is based on individual needs.

Group Living

Group Living Residences provide accommodation to people aged 18 and over. Group Living is for those who need care and support with daily living. Supports are offered in many group home and apartment settings. Personal Lifestyle Plans are developed yearly and reviewed regularly.
